project summary overview
The Mammograms Matter Health Initiative addresses the Centers for Disease Control’s Social Determinants of Health for at-risk individuals, cancer patients, cancer survivors, and caregivers within communities of color here in Fort Worth, Texas. Our goal is to reduce the impact of breast cancer in Fort Worth by emphasizing the importance of breast cancer awareness and education, especially for a community and group of individuals who have been historically marginalized which has led to worse health and cancer outcomes.
Meet Our Team
The Mammograms Matter Health Initiative relies on several important personnel to ensure the execution of objectives and proper dissemination of information to local community stakeholders and residents in the targeted area. These roles are vital to the effectiveness of the program and the communities we hope to serve. The Project Manager will plan, manage, and execute the initiative to ensure all benchmarks are met. The Medical Professional will serve as the subject matter expert; their expertise and background as a breast surgeon will be used to facilitate the health literacy education sessions and bring in additional speakers as needed. The Outreach Coordinator will be responsible for data entry, community engagement, and enrollment of residents for screenings and the health literacy forums. This position will also build rapport with city leaders and community stakeholders, introduce BarbaraCares, and offer ways of collaboration to address health disparities.

All screened mammogram participants were 40 years of age or older. Each screened participant made a verbal and written commitment to encourage two other women within seven days to get their mammogram and the importance of early detection. 114 mammograms were booked as a result of BarbaraCares outreach efforts. Through the direct appeal pledge from our mammogram recipients, we hope an additional 228 women as a result, will now schedule their mammogram on their own and/or reach out to BarbaraCares if there are any barriers to care. Screenings were administered by THR (Texas Health Resources) and Moncrief Cancer Institute.
Following these screenings, there were a number of women recommended for additional follow up and diagnostic testing. BarbaraCares is currently working with these individuals to provide further resources, and support for their continuum of care. Statistically, 1 in 8 women (12.5%) will be diagnosed with breast cancer over the course of their lifetime. Based on the original 100 target screenings for the Mammograms Matter Health Initiative and the national diagnosed average, our organization was bracing to provide wraparound services to an estimated 12 women, in the event there was a positive diagnosis. This initiative exceeded the 100-target mammogram allotment. With the additional 14 screenings provided, totaling 114 during the 9-month grant cycle, prospective wraparound services increased to 15 for those taking part in the health initiative. To-date, we are providing supportive services for 11 women diagnosed that were screened: about 10% of the women participating in the health initatitive.

Feedback from our caring conversation & the impact Of the sessions
The Mammograms Matter Health Initiative was more than just providing screenings and giving items away following a lecture. The success of this program would be found by creating more trust in the community with minoritized groups and building a foundational network of individuals that are committed to sharing learned information with their peers and social network groups. Increasing health literacy was our objective. Individuals like Ms. Brown, show our organization the power of training everyday people to become cultivators of change and health ambassadors/ advocates within their communities. Their involvement helps to change the trajectory of late-stage diagnoses and mortality rates. Each one, reach two!

One of our mammogram recipients shares her encounter with our newly hired Outreach Coordinator Mrs. Dunbar and how that experience led her to get a mammogram - it had been over 2 years since her last screening.
pASTOR mCDUFEY - HEAVENLY GOSPELOne of the host sites utilized for a mammogram screening outreach event. BarbaraCares held at least one screening event in each of the 3 identified targeted zip codes outlined in the grant proposal to address health disparities.
Stacey w.Mammogram event attendee hugs Mrs. Dunbar and thanks her for her persistence. Our Coordinator - Mrs. Dunbar scheduled Stacey several times but each time, she was unable to make the appointment due to work/family. It had been over 3 years since Stacey's last screening. Mrs. Dunbar understood the barriers that Stacey and so many others face but made sure she finally got checked.
